Please follow the directions on each document and also contact Apollo Freight at (310) 258-6148 x1 to obtain a Shipping Contract with rates and services.

• Credit Application - Complete if you wish to establish a line of credit
• Shipper Authorization To Screen Letter - Export Customers need to complete this letter
• Cargo Claims Policy and Procedures - Information and acknowledgment required
• Claim Notification Form - Complete in the event of loss or damage to a shipment
